'Continuity of care'

NEW LONDON, Wis.--ThedaCare at Home opened its fifth location this spring in the New London Family Medical Center in New London, Wis. The provider is no stranger to hospitals. Three of its five locations are part of hospitals or ambulatory centers. “It's a way for us to provide that continuity of care for patients being discharged from hospitals,” said Randy Lutz, ThedaCare's HME operations manager.
