UTICA, N.Y., and SYRACUSE, N.Y. - Upstate HomeCare will supply infusion therapy, oxygen and DME to patients of VNA Homecare as part of a new preferred provider relationship.

“Our organization is continually looking for ways to enhance our services and delivery of care for the communities we serve,” stated Greg LoPresti, CEO of Upstate HomeCare, in a press release.

The two companies are working on further developing innovative products for the community. One example: a care transition program designed to reduce hospital readmissions by presenting a more detailed, clear assessment of the patient's clinical status at the time of discharge.

Partnering with Upstate HomeCare, a “progressive post-acute care services company,” allows VNA Homecare to enhance delivery of care, says M. Kate Rolf, president and CEO.

“Their size and outreach can better support the needs of our patients and keep the cost of care more affordable,” she said in the release.

Upstate HomeCare and its 150 employees serve the Buffalo, Syracuse, Canandaigua and Rochester areas.

VNA Homecare's services include prenatal, rehabilitation, long-term and end-of-life care.
